The role of grandparents in upholding the continuity of families and communities, and in preserving cultural traditions, transcends national and cultural boundaries. This study, concentrating on the role of grandparenting among Maori grandparents in New Zealand, sought to illuminate the meanings and functions of grandparenthood, thereby fostering a conversation regarding its significance in various cultures worldwide. Eighteen Māori grandparents, and great-great grandparents, residing in intergenerational homes in Aotearoa New Zealand, were interviewed. A phenomenological investigation was undertaken to understand the data. From a Maori grandparent Elders' perspective, five themes emerged, illuminating the significance of their roles. These themes include: cultural responsibilities; support systems, resources, and assets; sociopolitical and economic obstacles; the current status of Elders' roles within families; and the intrinsic rewards and benefits of grandparenthood. Grandparents' support systems are analyzed, culminating in implications and recommendations for a more systemic and culturally responsive approach.
Dementia screening measures, standardized and crucial, are essential for geriatric care in the rapidly aging South-East Asian region. The Rowland Universal Dementia Assessment Scale (RUDAS), used in the Indonesian setting, has not yet established its cross-cultural transportability. This investigation sought to determine the reliability and validity of the Rowland Universal Dementia Assessment Scale (RUDAS) scores when applied in Indonesia. In a study involving nine neurologists and two geriatric nurses, along with 35 community-dwelling older adults, the Indonesian translation of the RUDAS (RUDAS-Ina) was administered to 135 older adults (52 male, 83 female) at a geriatric nursing center, whose ages ranged from 60 to 82. To validate the face and content, we utilized a consensus-building process. Following the confirmatory factor analysis, a single-factor model was evident in the outcomes. For research purposes, the reliability of scores from the RUDAS-Ina was only marginally satisfactory, as indicated by Cronbach's alpha (0.61). A multi-level linear regression model was applied to explore the association of RUDAS-Ina scores with age and gender, demonstrating a trend of lower RUDAS-Ina scores in individuals of older age. In opposition, there was no meaningful correlation between gender and the association. The findings point to the necessity of locally creating and validating items with Indonesian cultural sensitivity, a study that could be expanded to other Southeast Asian nations.

Patients with locally advanced gastric/gastroesophageal cancer, receiving neoadjuvant ICI-based therapy, were included in our research. We comprehensively reviewed PubMed, Embase, the Cochrane Library, and abstracts from significant international oncology conferences. Our meta-analysis was executed using the META package in R version 36.1.
Researchers identified 21 forthcoming phase I/II trials involving 687 patients. The pathological complete response (pCR) rate was 0.21 (95% confidence interval 0.18-0.24), the major pathological response (MPR) rate was 0.41 (95% confidence interval 0.31-0.52), and the R0 resection rate was 0.94 (95% confidence interval 0.92-0.96). The strongest efficacy was observed when ICI therapy was combined with radiochemotherapy, the weakest with ICI alone, and an intermediate efficacy was demonstrated when ICI was used alongside chemotherapy and anti-angiogenesis agents. The management of 20mm non-functioning pancreatic neuroendocrine tumors (PanNETs) is a topic of intense debate and lack of consensus. The differing biological compositions of these tumors create a dilemma in opting for either surgical removal or a strategy of watchful observation.
To evaluate the usefulness of preoperative radiological imaging and serum markers in determining the optimal surgical strategy for non-functioning pancreatic neuroendocrine tumors (PanNETs), a multicenter retrospective study was conducted. The study involved 78 patients (20 mm or less) who underwent resection at three tertiary care centers between 2004 and 2020. Enhancement computed tomography (CT) scans exhibited a non-hyper-attenuation pattern, characterized by hetero or hypo-attenuation, and implicated main pancreatic duct (MPD) involvement. Serological testing indicated elevated serum elastase 1 and plasma chromogranin A (CgA).
A significant subset of small, non-functional PanNETs, specifically 5 out of 78 (6%), showed evidence of lymph node metastasis; 11 out of 76 (14%) were categorized as WHO grade II, and a further 9 out of 66 (14%) displayed microvascular invasion. Consequently, 20 out of 78 (26%) of these PanNETs exhibited at least one of these high-risk pathological indicators. Assessment of patients before surgery showed hetero/hypo-attenuation in 25 of 69 patients (36%), and MPD involvement in 8 of 76 cases (11%). Elevated serum elastase 1 was found in a percentage of 3% (1 patient out of 33), whereas elevated plasma CgA was not observed in any of the 11 patients examined. The findings from multivariate logistic regression suggest a substantial correlation between hetero/hypo-attenuation and high-risk pathological factors, with an odds ratio of 61 (95% confidence interval 17-222). Likewise, the study demonstrated a significant connection between MPD involvement and high-risk pathological factors, with an odds ratio of 168 (95% confidence interval 16-1743). Radiological indicators, suggestive of concern, when combined, accurately predicted non-functioning pancreatic neuroendocrine tumors (PanNETs) with high-risk pathological characteristics, exhibiting approximately 75% sensitivity, 79% specificity, and 78% accuracy.
