A study assessed the dependability and accuracy of Rowland Universal Dementia Assessment Scale (RUDAS) results in the Indonesian population. A consensus on the optimal management of 20mm non-functioning pancreatic neuroendocrine tumors (PanNETs) has yet to be reached. The biological variability inherent in these tumors creates a conundrum when considering the options of resection and observation.
Our multicenter, retrospective cohort study, encompassing 78 patients who had undergone resection of non-functioning PanNETs (20mm or smaller) at three tertiary care centers from 2004 to 2020, investigated the effectiveness of pre-operative radiographic features and serum biomarkers in identifying appropriate surgical indications. Computed tomography (CT) scans revealed a non-hyper-attenuating pattern (heterogeneous/hypodense) on contrast enhancement, along with evidence of main pancreatic duct (MPD) involvement. Elevated serum elastase 1 and plasma chromogranin A (CgA) levels were also detected in blood tests.
Among small, non-functional PanNETs, a proportion of 5 out of 78 (6%) exhibited lymph node metastasis, while 11 out of 76 (14%) were classified as WHO grade II, and 9 out of 66 (14%) displayed microvascular invasion. Importantly, 20 out of 78 (26%) presented with at least one of these high-risk pathological features. During preoperative evaluations, hetero/hypo-attenuation was observed in 25 of 69 patients (36%) and, separately, MPD involvement was seen in 8 patients (11%) of the 76 evaluated. Elevated serum elastase 1 levels were present in 1 patient (3%) out of a total of 33, and plasma CgA levels were not elevated in any of the 11 patients (0%). The findings from multivariate logistic regression suggest a substantial correlation between hetero/hypo-attenuation and high-risk pathological factors, with an odds ratio of 61 (95% confidence interval 17-222). Likewise, the study demonstrated a significant connection between MPD involvement and high-risk pathological factors, with an odds ratio of 168 (95% confidence interval 16-1743). A pairing of two worrisome radiological features proved highly predictive of non-functioning PanNETs characterized by high-risk pathological factors, achieving a sensitivity of roughly 75%, a specificity of 79%, and an accuracy of 78%.
